CDPAP Eligibility Criteria
Who can be a caregiver?
- Children over 18 years old
- Relative
- Friend
- Legal Spouse
- Designated Representative
- Must be at least 18 years old and be able to work in the US legally.
To participate in NYHC’s CDPAP program the consumer must be:
- Live in New York State
- Active or eligible for Medicaid coverage
- Able to self-direct their own care or have an advocate that can act in this capacity by making decisions concerning the services being provided
- Have medical necessity for assistance with daily activities
- Currently receiving or eligible for long-term home care through a managed long-term care program (MLTC).
Consumer's Role Under CDPAP
- Recruiting, interviewing, hiring, and dismissing Personal Assistants
- Setting a schedule and determining how care is provided to meet individual requirements
- Notifying Fiscal Intermediary in case of any changes in details, particularly concerning the consumer's or personal assistant's name, address, or telephone number

Are CDPAP services free of cost?
Yes, as a consumer you will not have to pay for CDPAP services. CDPAP is a New York State Medicaid sponsored program, and upon approval you will not need to pay for services.
Can a friend or family member be my Personal Assistant?
Yes, friends and family can generally be hired to become your Personal Assistant.
Do my Personal Assistant’s need specialized training or certifications?
Personal Assistants are not required to be certified or undergo prior training.
